I know this is probably a really dumb question but, do you have Icon Tweaker? If not, you have to have it in order to use the pack. It's not designed for Icon Packager. Just google for Icon Tweaker, and you'll spot the latest version of it, which I believe is 1.2. I don't think it's in further development so I'm pretty sure the latest is the last, or so it would seem. If you do have it, I honestly don't know, you're the first person to have problems with it so far.
I highly doubt it's anything you're doing or not doing though.
Once you have the app installed, you should be able to click the .itt file and it'll launch Icon Tweaker, and apply it for you.
